What we help with

The conditions behind “I’ve tried everything.”

Seven of the most common reasons patients arrive at Twin Cities Metabolism — and how we evaluate the cascade behind each one before recommending a single intervention.

A different way of looking at conditions

Most chronic conditions are cascades, not categories.

Conventional care names conditions by their loudest symptom: low thyroid, PCOS, perimenopause, IBS. Useful labels — but they hide a more important truth.

In our clinic, almost every patient we see has two or three of these conditions at once, driven by an overlapping set of upstream causes: gut dysfunction, undertreated thyroid, HPA axis stress, insulin resistance, hormonal disruption, hidden inflammation. The label changes; the underlying physiology often doesn’t.

That’s why our evaluation starts with the cascade — not the diagnosis. We measure first. The right protocol falls out of the data.

Seven common entry points

What we actually work on.

Each card is a real case profile — the diagnostic frame, the markers we run, the questions we’re answering. Click any to see the cascade.

WHEN THE SAME PROTOCOLS STOP WORKING
Weight Loss Resistance
Adaptive thermogenesis, blunted satiety, post-diet downshift. Tested through metabolic rate, body composition, and hormonal markers — not the scale alone.
WHEN THE METABOLISM STOPS RESPONDING
Metabolic Damage
Years of chronic dieting blunt the engine. We measure RMR, T3/rT3, and the downstream signaling that explains why “eat less, move more” stopped working.
BEYOND THE TSH-ONLY SCREEN
Low Thyroid + Hashimoto’s
Nine thyroid markers, not one. Free T3, reverse T3, TPO, Tg, antibodies. The full pathway tells the story TSH alone never can.
THE METABOLIC ROOT OF CYCLE DISORDERS
Insulin Resistance + PCOS
Fasting insulin, HOMA-IR, glucose-tolerance, androgens, SHBG. PCOS is downstream of metabolic dysfunction more often than it is a cycle problem.
SEX HORMONES · HPA AXIS · METABOLITES
Hormone Imbalance
DUTCH-evaluated. We see the full hormone cascade — estrogen metabolites, cortisol slope, progesterone — not just one snapshot of one molecule.
MULTI-AXIS EVALUATION, NOT JUST HRT vs. NO HRT
Perimenopause + Menopause
Estrogen, progesterone, thyroid, adrenals, metabolic markers. The 40+ transition is a system shift — we treat it that way.
GUT TESTING THAT ACTUALLY TELLS YOU SOMETHING
Food Intolerance, IBS, SIBO, Leaky Gut
GI-MAP stool. Lactulose/mannitol intestinal permeability. SIBO breath test. Then the 5R protocol — sequenced, not stacked.

How we evaluate them

Same playbook regardless of the entry point.

Whichever condition brought you in, the diagnostic path runs through the same three tools. Each catches a different layer of the cascade.

DIRECT MEASUREMENT

RMR Test

A 20-minute Breezing Med breath test that measures your actual metabolic rate. See the RMR Test →

BODY COMP + CELLULAR

InBody Scan

Multi-frequency BIA: composition, cellular phase angle, hydration, grip strength. See the InBody Scan →
