When you book a consultation, complete an intake form, sign up for an assessment or the Reset program, or contact us, we collect the information you provide — including your name, email, phone, address, date of birth, health history, lab results, and other information relevant to your clinical care.

Information collected automatically. When you visit our website, we automatically collect standard log information (browser type, device, IP address, pages visited, referring URL, timestamps). We use standard analytics tools to understand how visitors use the site.

Cookies and tracking technologies. Our website uses cookies and similar technologies to remember preferences, measure traffic, and improve performance. You can configure your browser to refuse cookies, though some site functions may not work.

2. How we use information

We use industry-standard physical, technical, and administrative safeguards to protect the information we collect. However, no transmission or storage system is perfectly secure. If we learn of a security breach, we will notify affected individuals as required by law.

Marketing communications. You can unsubscribe from marketing emails at any time using the unsubscribe link or by contacting us. Service-related communications (appointment confirmations, lab results, treatment plan messages) will continue.

Health records. Access to your health records is governed by HIPAA and Minnesota law. Contact us to request a copy of your health records.

Our website is not directed at children under 13. We do not knowingly collect personal information from children under 13 without parental consent.
